Scope of Work

Purpose

A focused medical and aeromedical consultation designed to help the Client better understand how the Client's medical history may intersect with aviation, occupational, military, and other administrative medical standards so the Client can make informed decisions and identify appropriate questions for relevant professionals.

Included Services

• Advance review of the Client's medical summary and other agreed records.
• Individualized medical and aeromedical assessment of issues within the agreed scope.
• Education concerning relevant publicly available medical, FAA, military, occupational, and related standards, as appropriate to the Client's circumstances.
• Identification of documentation gaps, clinical questions, or issues the Client may wish to address with treating clinicians, an AME, legal counsel, or an accredited veterans representative.
• One individual consultation to discuss Daymark's observations, answer questions within scope, and identify practical next steps.
• A focused verbal discussion of Daymark's observations, relevant considerations, and practical next steps. No individualized written medical or aeromedical report, opinion, or summary is included in this consultation.

Not Included

• Preparing, presenting, prosecuting, filing, or pursuing a VA benefits claim.
• Choosing which VA disabilities or conditions the Client should claim or completing VA claim forms.
• Drafting advocacy statements, nexus opinions, disability benefit evidence, or submissions for use before VA unless separately contracted and legally permissible.
• Legal advice or representation.
• A treating medical evaluation, treatment plan, prescription, or ongoing clinical care.
• An FAA medical examination or issuance of an FAA medical certificate.
• A guarantee concerning VA benefits, FAA certification, military flight status, waiver, retention, employment, insurance, or other outcomes.

VA Accreditation Boundary: Daymark's consultation must retain genuine medical or aeromedical value independent of a VA benefits claim. If the Client requests individualized assistance whose purpose is to prepare or make a VA benefits claim ready for filing, Daymark will stop that portion of the work and refer the Client to an appropriately VA-accredited representative.

Client Deliverables / Pre-Work

• Completed Client Consent & Disclosure Package.
• Medical summary and agreed supporting records submitted through the designated method.
• A concise statement of the Client's aviation, occupational, transition, and consultation goals.
• Disclosure of material prior FAA, military medical, VA, or other administrative determinations relevant to the consultation.

Timing and Scheduling

Daymark will begin substantive review after receipt of the required fee and reasonably complete intake materials. Consultation timing and expected turnaround will be confirmed at scheduling. If the Client submits substantial additional records after review has begun, Daymark may request additional time or propose an expanded scope.

Cancellation / Rescheduling

Payment is generally nonrefundable once received. One reschedule is permitted without charge with at least 24 hours' notice. Late or additional rescheduling is at Daymark's discretion. No-shows forfeit the consultation and fee. Additional terms are provided in the Client Services Agreement.
